Adolescent-Centered Environment Assessment Tool

BACKGROUND
The Adolescent Centered Environment Assessment Process (ACE-AP) was developed by the University of Michigan's Adolescent Health Initiative and is a compilation of best practices in adolescent care. The ACE-AP is informed by standards of care set by the Society for Adolescent Health and Medicine, the Centers for Disease Control and Prevention, The American Academy of Pediatrics, the American Academy of Family Physicians, the American Congress of Obstetricians and Gynecologists, the United States Preventive Services Task Force, the World Health Organization, and AHIs youth council.
The purpose of the ACE-AP is to identify a health centers strengths in providing high-quality care to adolescent patients and to prioritize opportunities for improvement. The ACE-AP is comprehensive self-assessment tool, spanning 12 key areas of adolescent-centered care. An AHI coach facilitates the ACE-AP process with the participating health center team. During the process health centers prioritize areas for improvement over the course of the year. Based on those priorities, AHI creates a customized Implementation Plan with recommendations, work flows, scripts, and curated resources to help remove barriers to change and streamlines implementation to improve care delivery to adolescents.
