BACKGROUND

The Psychiatric Collaborative Care Model Systematic Case Review Tool allows the collaborative care (CoCM) treatment team to systematically review treatment progress and care management needs for the patients enrolled in their caseload. The behavioral health care manager (BHCM) will document key information for each patient. The BHCM and psychiatric consultant will use the resulting summary table and reports to ensure every patient receives appropriate stepped care.
